Quality Management- Performance Improvement Coordinator

Community Bridges, Inc. (CBI) is the premiere non-profit fully integrated healthcare provider of substance use and behavioral health programs in Arizona, including prevention, education and treatment using cutting-edge, nationally recognized, evidence-based models.

Our Quality Management Department is currently recruiting for a QM Performance Improvement Coordinator.

This position is responsible for projects assigned by the Performance Improvement Manager. Responsibilities include assisting with agency wide performance improvement. In conjunction with department leadership, selects and monitors relevant performance measures to ensure continuous quality improvement. Assists with accreditation and attends meetings when necessary at statewide provider sites. Collaborates with other departments or outside entities to ensure effectiveness of interventions.


  • Aligns agency performance measures and interventions with community stakeholders and regulators.
  • Serves as project manager for assigned quality initiatives across the agency including incentive payments, gaps in care, improving audit outcomes, improving HEDIS measures and others identified by department leadership.
  • Uses data reports and dashboards to identify areas in clinical operations and other departments that have opportunity for improvement. Collaborates with other department leadership on developing, implementing and monitoring interventions for effectiveness and makes adjustments based on quarterly data reports.
  • Consistently used the “Plan Do Study Act” (PDSA) model and conducts root-cause analysis (RCA) on target areas to support intervention development and testing. May use other quality improvement best practice tools depending on the nature of the project assigned.
  • Assists PI Manager with the design of internal performance incentive programs to encourage continuous quality improvement across like settings and programs.
  • Presents relevant data and trending at monthly standards and compliance meetings and other leadership meetings as assigned.
  • Helps monitor agency deliverable database for improvement opportunities by individual, program and health plan.
  • Helps with the design and maintenance of performance measurement dashboard and supports PI Analyst and IT department by providing clinical expertise.
  • Protects each patient’s confidentiality within the parameters of federal guidelines and established policy and process.

Required Reporting

  • In conjunction with data analysts, assists with preparing reports on a regular basis for agency leadership.

Training and Technical Assistance

  • As assigned by the Performance Improvement Manager or Director of Quality Management, provides training to groups on performance improvement interventions and measures.

External Audit Preparation

  • Assists the Director of Quality Management and Clinical Operations Department in preparing for CARF inspections and regular monitoring and reporting of compliance with all CARF standards, as instructed by the Director of Quality Management.
  • Responsible for assisting the Manager of Quality Management with preparing for any RHBA, Grant or other stakeholder audits.
  • Other duties as assigned by supervisory and administrative support.

Medical Records

  • Responsible for employing practices and performance that are complaint with the confidentiality regulations set forth by the Code of Federal Regulations, Arizona Administrative Code, Arizona Department of Health Services/Department of Behavioral Health Services Provider Manual, Arizona Office of Behavioral Health Licensure, the Commission on Accreditation of Rehabilitation Facilities, and Community Bridges Policies and Procedures.
  • Other duties as assigned by supervisory and administrative support.


  • Minimum of a Bachelor’s degree in an appropriate field.
  • Masters degree or CPHQ (or willing to obtain) preferred.
  • 1-2 years of project management experience preferred.
  • Must have a valid driver's license and clear 39 month MVR.

Experience Preferred
  • CPHQ (or willing to obtain) preferred.
  • 1 - 2 years: Project Management skills

  • Bachelors or better in Health Administration or related field
